This document sets out Herefordshire CCGs palliative and end of life care strategy for Children, Young People, Adults, and Older People. From diagnosis, through to end of life care and bereavement support, delivery of this vision will ensure that people will be supported, in the context of their lives, to live good lives and die well irrespective of their care setting.
