LET'S PLAN HEALTH AND CARE IN YOUR COMMUNITY

Herefordshire CCG are undertaking activities and events to listen to the views and experiences of local people, regarding community health and care services. This exercise is running until the end of September 2017.

We are seeking your views on what local community health and care services could look like in Herefordshire. We want you to be involved in how we shape services for the future.

RESULTS SO FAR

To date we hosted one patient participation forum (PPG) event at the CCG Offices on Wednesday, 12 July.

Additional engagement across Herefordshire includes: GP Practice visits, online survey and pop-up events in the town centre, library and other community group venues.

GET INVOLVED: TELL US WHAT YOU THINK

The best way to get involved is to attend one of our Engagement Events

We are returning to each locality through the month of September to host public evening events to:

  • listen to your experiences of health and care in your community
  • learn more about the issues you raised
  • hear your thoughts on the reports so far

We will return in October to host public drop-in sessions and present the findings of our reports.

If you would like to be involved, but can't attend one of our public meetings there are still a number of ways to get involved and stay informed:

  • Invite one of our representatives to your patient or public group - contact This email address is being protected from spambots. You need JavaScript enabled to view it. or 01432 260618
